Community Health Worker (CHW)

Job Description

• Screening of children and pregnant lactating women in catchment area.
• Referral of identified malnourished children and PLW’s and establishing linkages with Nutrition Team
• Act as liaison between formal health system and community
• Register all identified PLW’s and children under five years.
• Coordinate with EPI for immunization of mothers against tetanus and children for all childhood diseases.
• Registration of pregnant mothers and referral for antenatal visits/care to health centers.
• Counseling of pregnant women to enhance significance of antenatal and postnatal visits
• Counseling of eligible couples regarding family planning
• Organize community by developing Mother Support Groups in their catchment area.
• Household visit for inter-personal counseling( monthly visits to every pregnant and lactating mother and children under two years for regular follow up and dissemination of compliance to breastfeeding, complementary feeding, hygiene, Universal Salt Iodization and hand-washing)
• Regularly arranging awareness sessions for beneficiaries regarding Hygiene and Sanitation measures and practices regarding breast feeding and complementary feeding
• Distribution of Micronutrient powder, Zinc Sulphate and iron/folic acid tablets and keeping records
